Open and Closed Rhinoplasty
Two techniques commonly used for performing nose surgery are the closed technique and the open technique. The closed technique is generally preferred for aesthetic rhinoplasty procedures as it leaves no visible scars. During this procedure, the ENT surgeon reaches the interior of the nose by making incisions within the nostril, after which the skin is lifted up and separated from the underlying bone and cartilage. The open technique is usually used during complicated nose surgeries, and may leave visible scars. It involves the use of incisions over the nostrils and through the columella.
Below is a general outline of the procedure that is usually followed:
• The patient is first administered anaesthesia. This can be either local or general, depending on the complexity of the nose surgery.
• Incisions are made to reach the underlying bone and cartilage of the nose.
• This bone and cartilage is reshaped according to the previously determined surgical goal. During this stage, bone or cartilage material may be grafted from different sources, to augment the nose.
• The nasal tissue is wrapped over the fresh framework, and the incisions are closed with dissolving sutures.
• A splint is placed over the nose to maintain its new shape.
• Porous materials and supports may be placed within the nose, to sustain firmness.
Rhinoplasty is generally performed as an outpatient procedure, and typically takes around one to four hours, depending on the complexity of the particular case.
