			<content:encoded><![CDATA[<p>The term ‘rhinoplasty’ is derived from the Greek words ‘rhinos’ which means ‘nose’, and ‘plasticos’ which means ‘to mould’. Rhinoplasty refers to surgery for enhancing the aesthetic appearance of the nose, reconstructing a nose damaged by injury, or improving nasal function. <span id="more-305"></span> Over the years, rhinoplasty has grown in popularity, with roughly about 4,000 individuals in the UK undergoing nose surgery for cosmetic reasons, annually.</p>
<p>Though nose surgery is generally very safe, complications may occur in some cases, for which a revision rhinoplasty may be advised. One of the most common of such complications is inadequate projection of the nasal tip. This problem usually arises in cases where the large dorsum or hump of a patient was reduced during nose surgery, without the tip being adequately supported by a cartilage graft.  In some cases, the dorsal hump supports the nasal tip. Hence, any decrease in hump size usually results in a droopy nasal tip.</p>
<p>Generally, reduction rhinoplasty can reduce the size of a dorsal hump, adequately enough, though it may not always be able to provide enough nasal tip projection. Additionally, after an ENT surgeon sculpts the lower lateral cartilages, they can expect further loss of projection of the nose tip. In many cases, patients with large dorsal humps undergoing reduction rhinoplasty would require some sort of augmentation, to ensure that their nose does not have an inadequately projected tip.</p>
<p>Inadequately projected nasal tips can usually be prevented with a thorough preoperative assessment, during which the extent of tip projection and septal support is evaluated by the appointed ENT surgeon. During this assessment, x-rays and CT scans may be used to assess the projection of the nasal tip as adequate, inadequate or excessive; and determine how much septal support is available in the nose. Depending on this assessment, an appropriate surgical plan can be formulated. Generally:</p>
<ul>
<li>Patients with inadequate tip projection and      good septal support will usually only require onlay cartilage grafts to      the nasal tip.</li>
<li>Patients with inadequate tip projection and      inadequate septal support will usually require columellar struts or      umbrella grafts.</li>

Following the procedure, patients usually experience a moderate level of discomfort, which can be controlled by pain medications. <p>During the recovery period, a patient’s nose may sometimes be a bit asymmetrical or uneven, but this is expected to change as the nose heals. Below is some advice generally given by ENT surgeons to quicken the recovery process:</p>

			<content:encoded><![CDATA[<p>Non-surgical rhinoplasty was developed at the beginning of the nineteenth century, by James Leonard Corning, a New York City based neurologist, and Robert Gersuny, a Viennese physician. They pioneered the technique of using liquid paraffin to increase the height of a collapsed nasal dorsum, which characterises saddle nose deformities. Though liquid paraffin displayed good corrective efficacy, <span id="more-299"></span> it proved to be biologically harmful, even causing paraffinoma or wax cancer, in some cases. This led to the abandonment of liquid paraffin as soft-tissue filler for the nose.</p>
<p>The twentieth century witnessed more research into non-surgical <a href="http://www.entlondon.co.uk">rhinoplasty</a> techniques. In the 1960s, soft tissue fillers made of medical-grade silicone gel began to be used in non-surgical rhinoplasty. However, silicone gels also proved to be biologically harmful, as the soft tissue filler often migrated from the nose to other parts of a patient’s face and body, causing medical complications like ulcers and granulomas. This led to cosmetic surgeons and ENT surgeons recommending the ‘microdroplet technique’, during which minute doses of silicone gel were injected in the nose, over multiple sessions.</p>

Each option has its respective merits and demerits, including variations in awareness levels, side effects, risks and costs. <span id="more-293"></span> As rhinoplasty is usually an elective, non-emergent procedure, ENT surgeons usually advise pre-operative testing to determine the suitability of the patient for different types of anaesthesia. In most cases, patients who are in good general health are allowed to select the type of anaesthesia they wish to use, from the various available options.   </p>

One of the main side effects associated with general anaesthesia is post-operative nausea, which may increase blood pressure levels, and discomfort. </p>
